MAKING A CLAIM

ALL CLAIMS
See the Studentsafe website for advice on making a claim. There are some additional tips below. If you still need help with your claim, contact the Insurance Office or call Studentsafe on 0800 486 004.

First download and print a claim form or pick one up from reception at Victoria International (level 2 Rutherford House).

Read the important information at the beginning of the form. Once you have completed the form, don’t forget to sign and date the back page, or your claim will not be processed. Sent the completed form, along with your receipts or invoices to the address at the bottom of the form. It’s a good idea to keep a copy of your receipts and invoices for your own records.

1. POLICY INFORMATION

For policy type, tick ‘University’
For Educational Body, write ‘Victoria University’

2. Person making the claim

fill in this section if you are making a claim for a family member. Fill in your details in section 1. and the family member’s details in section 2.

3. Details of claim

Write a brief description of why you are making a claim eg:
 “My baggage was delayed for two days when I arrived in Wellington on February 17. I had to buy toiletries and some clothes to use until my baggage was found.”
“I felt sick in the weekend and needed to see the after-hours doctor. I am claiming for the cost of the doctor’s visit and the medication I was prescribed.”

4. Medical expenses + dental claim

Answer all the questions. If a question does not apply to you, write “n/a”. List the amounts you have had to pay, or have been invoiced for.

Make sure you sign the Medical Authority at the bottom of this page. This gives the insurance company permission to find out more about your medical treatment from your doctor if they need more information. If you don’t sign it, they might not be able to pay your claim.

5. Baggage

Fill in this section if you are claiming for lost baggage, or for lost, damaged or stolen property.

Remember that there is an excess of $100 for personal property claims and $200 for personal computers. If the amount you are claiming is less than the excess, you will not receive any money.

6. Cancellation of Travel Disruption

This is for costs relating to missed flights or travel delays. You need to include as many of the listed attachments as possible (don’t include if not relevant to your claim).

7. All other sections

This is for any claims that don’t come under the categories above. Attach any information you think is relevant to your claim. If Vero needs more information, they will contact you.

MEDICAL CLAIMS
Answer all the questions that apply to your claim. If you have paid your doctor’s bill or prescription costs already, write the amounts you have paid in the box at the top of page 3, and attach the receipts/invoices. If you have not yet paid your bill, or are applying for pre-approval, list the costs you have been quoted in the box at the bottom of page two. If you have a written quotation, attach it to the form. It’s a good idea to make a copy of your receipts/invoices/quotations for your own records.

PERSONAL PROPERTY CLAIMS
This is where you claim for property that has been lost, stolen or damaged.
Remember that there is an excess of $100 for personal property claims and $500 for personal computers. If the amount you are claiming is less than the excess, you will not receive any money if your claim is approved. If you have reported the loss/theft/damage to the police, attach a copy of the police report if you have one, or give the report number.

OTHER CLAIMS
For any claim that is not for medical expenses or personal property, describe the circumstances of the claim in this section. ‘Costs or protection’ means what you hope the insurance will pay for. If you know the exact amount you are claiming, write it here and attach an invoices, receipts, or written quotes if you have them.

If you are claiming for baggage lost by the airline you need to attach a document from the airline showing how long your bags were lost, and receipts for any essential items you had to buy during that time.

If your claim is for emergency travel you must call the insurance company before you buy any tickets.

CLAIMING UNDER A FAMILY POLICY
In the ‘student details’ section of the form, write the details of the main policy holder, ie the student. Include the name of the family member who the claim is for when writing the claim description in the first box of the relevant claim section (Medical/Personal property/Other), and write that it is a family policy claim.
