All accidents, incidents, injuries and near misses are to be reported to the Senior Advisors - Health and Safety, Emergency Management and Business Continuity as soon as possible. You can send completed forms to email@example.com

Where a serious accident, incident, injury or near miss has occurred at work and it fits the definition of a Notifiable Event, these need to be reported immediately by contacting either Victoria's Senior Advisors - Health and Safety, Emergency Management and Business Continuity on 04 463-6053 or 04 463 6071; Security 04 463-6634; or the Emergency number 04 463-9999.
A Notifiable Event is defined in the Health and Safety at Work Act as follows:
25. Meaning of notifiable event
In this Act, unless the context otherwise requires, a notifiable event means any of the following events that arise from work:
(a) the death of a person; or
(b) a notifiable injury or illness; or
(c) a notifiable incident.
